Preface

This is the third edition of a book that was conceived in 1990 and first published in 1993. To indicate how much the world of health economics has changed, the first edition made a single reference to "managed care," and that was in the context of monopoly power in the provision of health care. In addition, the debate on the Clinton health plan illuminated a large set of issues that invigorated the discipline of health economics.

This book was first written to assist health economics instructors in developing a clear, step-by-step understanding of health economics for their students. We also believe that it is important for instructors to show what health economics researchers are doing in theory and in empirical work. The book synthesizes contemporary developments around a set of basic economic principles, including maximization of consumer utility (or well-being) and economic profit, wishing to make these principles accessible to undergraduate students, as well as to graduate students. Rather than organizing the book around the institutions specific to the health care economy, we have used core economics themes as basic as supply and demand, as venerable as technology or labor issues, and as modern as the economics of information. Moreover, we have sought to improve accessibility to the book for the wide range of health services students and practitioners whose knowledge of economics may be more limited.

We have followed the philosophy that students must have a working knowledge of the analytical tools of economics and econometrics to appreciate the field of health economics. Some students may be ready to plunge directly into Cost-Benefit Analysis (chapter 4) or The Production of Health (chapter 5) upon completion of the introductory chapter 1. However, chapters 2 (Microeconomic Tools) and 3 (Statistical Tools) allow the students and their teachers to develop or to review the needed analytical concepts before tackling the core subject matter. In chapters 2 and 3, students with as little as one semester of microeconomics may review and study how economists analyze problems, using examples that are relevant to health economics. No calculus is needed.

Consistent with an emphasis on clarity of exposition, this book makes extensive use of graphs, tables, and charts. Discussion questions and exercises are provided to help students master the basics and to prompt them to think about the issues. Features on up-to-date applications of theory and policy developments are also included, as well as the occasional tidbit containing purely background information.

Finally, we caution that some of the chapters, such as those on insurance, although devoid of advanced mathematics, may still require considerable effort. No painless way is available to appreciate the scope of the contributions that scholars have made in recent years. Sections or subsections that are more demanding and/or peripheral to the core material have been identified. These sections can be omitted without detracting from the flow of the book. More advanced students of the health care economy who wish to be challenged further can utilize a comprehensive references section, with more than 750 sources, so that their (and our) work can be enriched through referral to the original sources. What's Changed in the Third Edition?

It is critical that this textbook contain the most current knowledge and address the needs of our readers. As with the second edition, almost every chapter is revised to incorporate significant developments in the field.

In response to the instructors and students who use this book, we have moved the chapter on cost-benefit analysis forward, so that it is now chapter 4. We have made major changes in the treatment of insurance and managed care. Also in response to user demand, we have broken up a single health insurance chapter into two components. Insurance (chapter 7), which looks at the individual determinants of insurance, now precedes Consumer Choice (chapter 8), as well as a set of chapters on important, related economic topics. The Organization of Health Insurance Markets (chapter 11), which looks at the development of health insurance markets (and asks the question "who pays?"), precedes the entirely rewritten and greatly expanded chapter on Managed Care (chapter 12). In this chapter, we make a major effort to link theoretical models and empirical findings.

The section on governmental actions regarding the health economy has been reorganized. Chapter 20 looks at conceptual issues regarding government intervention in health care markets; chapter 21 investigates the principal regulatory mechanisms, including antitrust policy. These are followed by an updated chapter 22, which includes information about the 1997 changes to Medicare and Medicaid.

Finally, we finish the book with two new chapters. Chapter 25 looks at economic "bads." Economists like to look at things that are good for us, but some things such as cigarettes are not. Chapter 26 is an entirely new chapter on pharmaceuticals. Rather than a simple "industry study," we look at those portions of the pharmaceutical industry with particularly novel economic contents. These include questions as to the optimal combinations of drug therapies and other economic technologies, the appropriate amount of patent protection that an industry (in this case the pharmaceutical industry) should enjoy, and price discrimination by pharmaceutical firms (or why drugs are cheaper in Mexico than in the United States).

Another change involves the emergence of Internet resources for students and scholars. Tables and charts that were once available only in book form, and then only after several years, are now available much more quickly, and are often available on the Internet long before they are available (if at all) in print. The downside to this explosion of information involves editorial oversight (not all sources are good ones) and frustrating tendencies for Internet sites to disappear. We have chosen to focus on governmental sites such as the following for references that we believe to be both long lasting and reliable.

Alternative Course Designs

As a developing field, the economics of health and health care has an evolving body of literature, and there is no single "correct" order for the course design. Economists (particularly U.S. economists) typically organize topics through markets, with government roles coming much later. Students and scholars of public health assign the governmental sector far more importance; it is "public" health. Unfortunately, a text is necessarily linear in that one chapter has to follow another.

The Economics of Health and Health Care offers instructors considerable flexibility. We have divided the 26 chapters into nine parts:

Basic Economic Tools (chapters 1-4) Supply and Demand (chapters 5-8) Information (chapters 9-10) Insurance and Organization of Health Providers (chapters 11-12) Technology (chapters 13-14) Labor (chapters 15-16) Hospitals and Nursing Homes (chapters 17-18) Social Insurance (chapters 19-23) Policy Issues and Analyses (chapters 24-26)

The categories are not entirely exclusive. Chapter 7, looking at the demand and supply of insurance, is as important to part 4 on insurance as it is to part 2 on demand and supply of goods.

From front to back, the organization follows an "economics" model in which consumers and firms are first analyzed in a world without government and governmental policies. As a result, explicit discussions of government policies do not come until chapter 19, although regulation, licensing, mandates, and the like are discussed in reference to other topics much earlier. Many economics instructors may wish to follow the chapters in the order of the book.

Other instructors, particularly those who are interested in public health and in governmental policies, may wish to "tool up" on some of the earlier analyses, and then skip directly to parts 8 and 9, in which we look at social insurance, health care regulation, and health care reform. After that, they may wish to browse selected topics. Although some analyses build on each other within chapters, we have sought to minimize cross-referencing among chapters. 5th edition works in place of 6th edition

I bought this 5th edition book used for ~9.00 with shipping instead of spending the 150.00 for the new 6th edition. So far from what I can tell it is 99.8% the same- for the questions and text. I have had no problem following what the prof. has assigned and the material covered by using this book instead. Shipping was fast and book was in the condition as stated in the description, or better.

A pretty good book for what it is...

As with all economics textbooks and the subject of economics in general, this book is a bit dry. That said however, it's quite thorough in its coverage of how consumers, producers, and insurers make decisions about health and health care. It gives explanations that are rather simple to understand and goes in depth as far as would be considered normal for a 300 level elective course. I should probably note that my instructor is one of the book's authors, so take my review how you will.

This book is divided into 6 parts and 25 chapters. It provides an interesting introducton on issues related to the scope of health economics before discussing the important topic of demand and supply. I have found chapter 4 very useful since it provides a good insight of economic evaluation, with the discussion related to cost-benefit analysis, cost-effectiveness analysis, cost-utility and cost-minimization analysis. During this period characterized by the deficits of health care, the reader will find useful information in chapter related to health insurance, managed care, consumer preference, etc that provide knowledge required for planning and policy-making.

Jack Zipes is the translator of The Complete Fairy Tales of the Brothers Grimm (Bantam), the editor of The Great Fairy Tale Tradition (Norton), and the author of Grimm Legacies (Princeton). He is professor emeritus of German and comparative literature at the University of Minnesota. Andrea Dezsö is a visual artist who exhibits in museums and galleries worldwide and is associate professor of art at Hampshire College.

A Grand First Edition

If you love Grimms and aren't very fluent in German, this is a book to get excited about. Even if you are fluent, it's pretty exciting, too.

Over the years, one of the top questions I've received as SurLaLune is: "Where are the dark, gritty fairy tales I hear about?" Well, that's a complicated question, but one interpretation of what they ask is: "Where are those lesser edited Grimms' tales that I've heard about?"

For some reason, the entirety of the first Grimms' edition has not been translated into English previously. Zipes, in the Acknowledgements of this new book, says that during the Grimms' bicentennial in 2012 he decided, "if nobody was going to undertake this 'task,' I would do it--and do it out of pleasure and to share the unusual tales the Grimms collected as young men when they had not fully realized what a treasure they had uncovered."*

That's a boon since, after all, Zipes has also translated one of the most used and most recommended editions of Grimms. Nice to have Zipes' translations of both the earliest and later versions of the tales to compare and consider.

After all, the Grimms had seven editions of their famous collection and there were considerable changes between that first and seventh edition. And many of those earlier versions were grittier and more adult since the Grimms hadn't intended children to be one of their primary audiences.

From the book's introduction:

"In fact, many of the tales in the first editions are more fabulous and baffling than those refined versions in the final edition, for they retain the pungent and naive flavor of the oral tradition. They are stunning narratives precisely because they are so blunt and unpretentious. Moreover, the Grimms had not yet 'vaccinated' or censored them with their sentimental Christianity and puritanical ideology."

And, a bonus for scholars like me and some of you, Zipes also translated some of the Grimms' notes to the tales:

"As for the sholarly notes to the tales, I have provided a thorough summary of each note to indicate sources, and I have also translated the variants of the tale that I thought were important. These notes reveal, in my opinion, how knowledgeable and erudite the Grimms were at a very young age."

Finally, this means that the table of contents to the book is different from what we consider the standard contents since the Grimms removed and added tales over subsequent editions. Zipes has included several of those omitted tales in his standard Grimms in the appendices but now they are provided in their original context in their original place. For example, "The Summer and Winter Garden"--no. 68 in the first edition and a Beauty and the Beast tale--was omitted in later editions for being too French. In later editions, KHM 68 (the way Grimms tales are referenced by number, abbreviating the German title Kinder- und Hausmärchen with the tale's number in the contents) is "The Thief and His Master."

And in this edition, you get the infamous and disturbing "How Some Children Played at Slaughtering." In later editions, you get KHM 22 as "The Riddle" instead which is much more innocuous as the titles imply.

So, yes, I highly recommend this book for fairy tale fans.

*All my quotations from the book are from the unproofed ARC I received several weeks ago. I waited for the final review copy to arrive but the envelope arrived last week split open along a seam and empty with a nice sticker on the front from USPS telling me that my package "Arrived Without Contents." I'm not sure if and when another will arrive.

Grimms' Tales as they were meant to be read

For the first time one is able to read the original editions of the famous tales of the Brothers Grimm in English in an excellent translation by Jack Zipes. Published originally in two volumes in 1812 and 1815, they represent the "raw material" (and it is often raw) that was later edited, polished and bowdlerized to be more acceptable to polite society and children. Almost all of us will be familiar with many of the tales as they were later presented culminating in the 7th edition in 1857, which is often seen as definitive.
Jacob and Wilhelm Grimm were interested in 'recovering the "true" nature of the German people through their so-called natural Poesie, the term that the Grimms often used to describe the formidable ancient Germanic and Nordic literature.' (p. xxiii). Rather than a work of entertainment for children this collection was intended as a scholarly work and included extensive scholarly notes. It was presented with minimal editing with the aim of capturing the original stories as they had been told to the brothers by a variety of sources. Zipes explains, in an interesting introduction, how they wanted to show how the cultivated literature, Kunstpoesie, evolved and eventually replaced Naturpoesie (tales, legends etc.) which survived in oral traditions.
There are a total of 156 tales in the two volumes, including nonsense stories, fables, animal and magical stories. All the well-known stories are there - Rapunzel, Snow White, Bluebeard, etc., etc. - but usually in shorter and often more basic forms. For example, in one version of Rapunzel her meetings with the prince are revealed when she gets pregnant and her clothes become too tight. The tales are often also blood thirsty; one in particular is #22 of Volume 1, "How Some Children Played at Slaughtering", where children pretend to be a butcher by slaughtering a playmate in one version or a sibling in another. As Zipes points out, they are tales about "wounded" young people, people who are exploited or in need and abuse of power but also often about wicked people getting their just desserts.
As the aim was to capture stories they had been told, there is quite a lot of repetition of stories and themes: three sons with the youngest seen as not too smart but in the end winning out; leaving instructions NEVER to open a particular door; princes and princesses trapped as animals and so on. However, it is interesting to see the variations around a basic theme and how the stories changed in telling and re-telling. It is also noteworthy to see how these themes have continued over history, particularly the large number of stories of metamorphosis, an area of great interest to Ovid.
My German is not good enough to comment on the translation but the English is very readable and maintains an air of oral story telling. Zipes also includes a summary of the original notes along with sources when known. Throughout the book are illustrations by Andrea Dezsö, who uses a style reminiscent of Shadow Theater that adds a great deal to the book.
This collection is fun and sometimes shocking to read and certainly not the children's book of the later editions. Making this original work available in English shows the significance of the Grimm brothers as academics in philology rather than merely tellers of children's fairy tales.

The original edition of the tales from the Brothers Grimm

This is a wonderful volume. As it were, this represents the first version of the Brothers Grimm Fairy Tale collection. The author is Professor Emeritus of German and Comparative Literature at the University of Minnesota. As such, he wondered at the lack of a published version of the original volume put together by the Brothers Grimm. Consequently, Dr. Zipes translated and edited the 1812/15 version and it was published by Princeton University Press.

The Introduction by Zipes places the volume in context; he discusses the goals of Jacob and Wilhelm Grimm; he notes the changes in the tone and tenor of the stories over multiple editions of the collected works. If one is familiar with the story of Rapunzel, the changes are rather stark. I(n the original version, she becomes pregnant by her visiting male friend. In later versions, pregnancy did NOT occur.

The Grimm brothers' prefatory comments on Volume 1 and Volume 2 are provided in this collection, adding their own personal observations to the picture. Their desire to bring the collective tales told and retold by people is manifest in the publication of their original version.

But it is the stories that are the highlight here. There are two frog prince stories that I read (# 1 in Volume 1 and # 13 in Volume 2). In each case, the princesses were rather snotty to the frog. An edgier version as compared with subsequent volumes. One of the issues raised by the Brothers is that lessons were often taught in these tales. One is that one is rewarded for good and generous deeds. # 83 in Volume 1, "The Poor Maiden." This is the story of a poor maiden who gave away what little she had--including clothing--to help others. The end? The stars fell from heaven and became coins that she used to become "rich for the rest of her life" (page 263).

It's been a long time since I read "Puss in Boots" (Volume 1, # 33). The cat who wore boots and did great deeds for his master ended up producing a royal wife for the man, a kingdom, too, at a later point. And, finally, the cat became prime minister. Other familiar stories with somewhat edgier approaches--Little Red Cap (Volume 1, # 26), or, as we know it, "Little Red Riding Hood; Hansel and Gretel (# 15, Volume 1); Rapunzel (already noted (# 12, Volume 1). And so on.

This is a volume well worth acquiring, to provide a context against which to compare later volumes and more familiar versions of the tales that we are aware of.

Very comprehensive

[[VIDEOID:mo3LJM3H8C0WMJ0]]Someone from the Resources Development Administration (RDA) labs has leaked this confidential report. Not only that, he/she has successfully sold this online (where I bought mine) and at major bookstores! That's one smart, profiteering, mole.

This is the fact book on Pandora, the kind of book issued to soldiers who are about to invade that planet for its precious resources. It has all the information on the ecosystem, resources, flora, fauna and the native population called Na'vi, complete with all their culture and behaviour. There are classified information on the weapon systems used by RDA, including the Avatar project. And everything is accompanied by CG images.

I had some hesitation before buying this survival guide and it's not because I'm afraid that James Cameron might send Avatars to get me. I like art books and books on movie production, this book is neither of those. This is really a visual companion to all the stuff that's shown in the movie. It's for the ultimate fans, those who need to know every little detail, from the ferns on the ground to the mating habits of the Na'vi.

It's an interesting book but recommended more to the hard core fans of the movie.

Excellent comprehensive guide to the world of Avatar.

If you have seen the movie and had and/or developed lots of questions about Pandora, the Navi, Earth's current state (in regards to the film),etc or if you just want everything fleshed out for you, then this book is for you.

It's VERY comprehensive. It's almost as if one has a textbook of everything and anything about Pandora. I love it. To see how much detail has been given to everything (and I literally mean everything) just makes me sit in aw of what James Cameron has created and the world that he (with the help of others) has developed. Everything just comes to life and so much detail is put into everything that Pandora could easily exist.

If you are looking for a book that has some sort of "making of" material in it, then you won't find it here. There is nothing related to making of the movie in the book.

From now on when I watch the movie, I am going to enjoy it a lot more knowing all the specific little details behind the story and Pandora. It just helps me enjoy the masterpiece that is Avatar even more.

Don't hesitate to buy it. There is a narrative to the book at the beginning but then it divides itself up like a textbook (although there is commentary by a specific individual within each section). I think it's great for all ages.

The Triumph of Odysseus is part of the highly successful Reading Greek series. The book can be used by anyone who has completed Reading Greek or is at an intermediate or advanced stage of ancient Greek and it is ideal for use with students in the upper forms of schools, at university and in summer schools and weekend courses.

Oh, Beast, what a guy!

I loved the relationship between Beauty and Beast! Both characters, as well as the minor players, were interesting and "real". I'm not usually a fairy tale kind of reader, but I really enjoyed BEAST CHARMING.

What do you do when your father schemes every step of the way to get HIS happily ever after? Beauty's father is a liar and a cheat. He has tried her whole life to use his daughters to gain riches in the world. When the novel opens, we find Beauty working for a temp agency. Will her next assignment be the one that will free them both from his grasp?

Beast Charming is a great twist on the classic Beauty and the Beast fairy tale. This book stays true to the overall aspects but adds enough twists and turns to make it memorable. Wardell's writing and whit are unmatched in any retelling I've encountered so far. Her characters are REAL and exaggerated at the same time.

You will fall in love with Waverly, Beast, Beauty, Grace, Manny, and Steve. The banter. The scenery. And if you've already read Fairy Godmother's Inc. you'll see nods to that story as well. I cannot wait to see what the author comes up with next. This story is about finding your happily ever after regardless of what someone looks like or is perceived in being.

Loved reading this book. Lots of LOL moments. I give it 4 stars!

Brought a new life to an old fairytale! Full of wit, charm & LOL humor mixed in!

Oh my goodness. This author just continues to blow me away. Jenniffer Wardell has this amazing, witty sense of humor that interweaves into her storytelling. Her writing just flows effortlessly for me. Beast Charming was thrilling, refreshing, full of wit, charm, & an amazing bit of humor all mingled together. It brought about new life to an old, classic fairytale!

The characters were stunningly crafted! I'm actually having an internal debate about whether or not I loved this version of Beauty & the Beast more so than the original cartoon... Yes... it was THAT good.

Reading Jenniffer's guest post above (THANK YOU FOR THAT JW!), you'll immediately get a sense of what I mean when I say that the characters are perfect. Beauty is strong, independent, and oh so smart-mouthed. Hehe. I loved her! It's so nice to see a female lead who isn't afraid to take charge and go after what she wants. She's lived a horrible life thanks to her father pushing her on every prince & single noble man within a million mile radius. Her & her sister... I don't envy them for being the laugh of the ball at times. But they kept their heads high & they finally managed to get away from him to live their own lives.

Beast! I really, really loved his character. I liked the strength and the internal debate that he had going on within himself. He was a "beast" of a business man until he literally became, well... a beast. Then he shut himself out of the business world, not wanting people to see him looking more like his nickname. Waverly! Ah! I think he's truly one of my favorite characters in the entire novel. I loved every time he graced the pages. He had this calm sense of all-knowing about him that just hypnotized me.

The plot was great! I wasn't expecting some of the twists and turns that Jenniffer took while working through their story!

Overall, perfect 2nd novel by Miss Wardell. I can't wait to see what she comes out with next!!!!

He works on comparative democratization and has published widely on a range of topics including electoral violence, political parties, and power-sharing.
